What is acne?
Acne affects 80% of people between the ages of 18 and 30. It is a condition of the skin characterized by the overproduction of sebum, the oil that coats the face and scalp of mammals.
The purpose of sebum is to carry dead cells to the surface and to waterproof and protect the skin. However, too much sebum is produced and it collects in the pores, preventing bacteria from escaping.
This blockage leads to inflammation and eventually infection, causing white blood cells to flood the area. After a few days, dead white blood cells accumulate as pus and a pimple is formed.
The root cause of the pathology of acne is not well understood. Nonetheless, there is a clear correlation between endocannabinoid deficiency, meaning faulty endocannabinoid signalling, and the manifestation of various facets of acne, from excessive sebum, to excessive inflammatory response to skin irritation.
When the endocannabinoid system fails to function efficiently, it fails to regulate the cells in which the deficiency is occuring. In the case of acne, sebocytes in the skin appear to have low levels of endocannabinoid mRNA (the leftovers of genetic transcription), strongly suggesting that acne could be a “symptom” of endocannabinoid system deficiency.
For example, the sebaceous glands that produce sebum are loaded with cannabinoid receptors. Endocannabinoids, and all cannabinoids for that matter, are fat-based molecules. Sebocytes (the cells which comprise the sebaceous glands) exist for the sole purpose of producing a fatty substance: sebum.
As sebum is produced, so are endocannabinoids. These endocannabinoids continuously activate cannabinoid receptors, which continuously suppress the activity level of the cell (in this case, the activity is to produce sebum) to the appropriate level dictated by homeostasis. In cases of acne, there appears to be a significant correlation with underexpression of the endocannabinoid system.
Not only does CBD inhibit lipid production (lipids make up all fatty and oily substances), but it also acts as a buffer for the endocannabinoids in their activity on cellular transport proteins and receptors.
This study, carried out to determine whether or not topical CBD oil ointment was effective in treating acne, produced staggering results. After two weeks of daily use, 100% of respondents – every single participant in the study, without exception – reported positive change in the expression of their condition. 60% experienced complete elimination of acne.
How is CBD for acne so effective?

As we just discussed, acne is caused by abnormally high levels of sebum, an oil that helps to remove dead skin cells. The production of this oil, a lipid molecule, is mediated by CB2 receptors in follicle cells. Specifically, CB2 activation is correlated with increased levels of sebum production. When CBD, a negative modulator of this receptor, is applied to the skin via a topical lotion or administered systemically through an edible or sublingual drops, it blocks the action of the CB2 receptor partially, but still enough to effectively diminish the overproduction of this oil.
Another secondary mechanism of acne involves an inflammatory response to bacteria that accumulates in a sebum-filled pore. This is what produces a pimple, and CBD helps to block the over-recruitment of immune cells to this site, and to expedite the removal of waste from the area as well through vasodilation and increased cellular metabolism.
While the exact processes of CBD in its action against acne and many other conditions are still not crystal clear, each day we gain new insight into the world of our endocannabinoid system and how it regulates homeostasis in every different system of our body.
